Pathways to Well-Being and Continuum of Care Reform Learning Series
Pathways to Well-Being supports youth by bringing together youth, their family, Behavioral Health Services, Child Welfare Services, and other critical partners to support the safety, permanency, and well-being of youth with identified behavioral health needs. As a behavioral health service provider in the County of San Diego, your responsibilities under Pathways to Well-Being include your active participation in the Child and Family Team (CFT) and creating and maintaining complete and accurate documentation of service activities. This series of learning opportunities is designed to help you familiarize yourself with Pathways to Well-Being and Continuum of Care Reform concepts and practice.
Child and Family Team Roles and Responsibilities eLearning
Child and Family Teams are mandated by the state and are embraced locally in San Diego as a critically important way to support youth and families. This course is designed to help you understand Child and Family Teams and Child and Family Team meetings, comply with mandated Child and Family Team meeting timelines, and identify specific roles and responsibilities for each Child and Family Team member.

Pathways to Well-Being Documentation Microlearnings
These microlearning courses are designed to help Behavioral Health Services providers get just-in-time help with Pathways to Well-Being forms and other documentation, including how and when to access and complete them, along with helpful tips for each.

The Integrated Core Practice Model (ICPM), Pathways to Well-Being, and Continuum of Care Reform outline the shared values, core components, and standards of practice expected from the professionals who serve San Diego County’s children, youth and families who have intensive behavioral health needs and are involved with the Child Welfare System.
This family-centered approach seeks to improve collaboration and communication among Behavioral Health, Child Welfare, and Juvenile Probation staff, as well as the children, youth and families they serve. Understanding this shared vision will help you to ensure that these comprehensive, strengths-based, culturally relevant, and trauma-informed supports and services are responsive to the strengths and underlying needs of those you serve.
